Oral History Project · Pre-Shoot Agreement Imprinted recording consent & usage agreement This pre-shoot agreement records the interviewee's consent to the making of an oral history recording, the scope of permission granted to Imprinted for any subsequent use of that recording, and the fee and payment terms agreed for the shoot. Where this agreement is signed by a relative or other representative, the signatory confirms that it is completed with the interviewee's knowledge and permission. 1. Consent to recording This consent relates to the making of the recording. Permission for any subsequent use of the material is specified separately below. I confirm that the interviewee has given their consent for their oral history interview to be recorded by Imprinted in audio and/or video format. 4. Permission for use of the recording Please select one permission level. The selected permission will form part of this signed agreement. Unrestricted use The interviewee authorises Imprinted to edit, reproduce, publish, exhibit and distribute all or part of the interview, including audio, video, transcripts and still images, for the project and its related editorial, educational, archival and promotional purposes. Selected portions only Public use is limited to the material, subjects and conditions specified below. Any use outside the stated scope requires prior written approval. Restrictions or conditions governing public use Private recording only The recording may be made and supplied for private use only. No public release, publication, exhibition, promotional use, social-media use or other public distribution of the recording or any extract from it is authorised. 5. Fee and payment terms Please enter the fee agreed for the shoot. Unless otherwise agreed in writing, payment is due within seven calendar days following completion of the shoot. Agreed fee (£) I acknowledge the fee stated above and agree to pay it in full within seven calendar days following completion of the shoot, unless an alternative payment arrangement has been agreed in writing with Imprinted. 6. Attribution preference Where public use is authorised, please specify how the interviewee is to be identified. Declaration and agreement I confirm that I consent to my interview being audio and/or video recorded for the Imprinted oral history project. I understand that my separate usage choice on this form determines whether Imprinted may publish or otherwise use any part of the recording. Where unrestricted use is selected, I give Imprinted permission to edit, reproduce, publish, exhibit and distribute all or part of the interview, including transcripts, audio, video and still images taken from the recording, for the purposes of the Imprinted project and its related editorial, educational, archival and promotional outputs. Where selected portions only is chosen, Imprinted may use only material falling within the scope or restrictions stated on this form, or material I approve in writing before publication. Material outside that permission is not authorised for public use. Where private recording only is chosen, I consent to the recording being made and supplied as a private recording to keep. I do not authorise public release, publication, exhibition, promotional use, social-media use or other public distribution of the recording or extracts from it. I agree to pay the agreed fee stated on this form within 7 calendar days following completion of the shoot, unless a different payment arrangement is agreed in writing with Imprinted. I confirm that the information, fee and consent choices I have provided are accurate and that this electronic signature represents my agreement to this form. I confirm that I have read and understood this agreement and that the permissions, agreed fee and payment terms recorded above accurately reflect the agreement made with Imprinted. 8.
